Information regarding disclosure of your Social Security Number under Public Law 93-579 Section 7(b): Solicitation of the Social Security Number (SSN) is authorized under
provision E.O. 9397, dated November 22, 1943. The disclosure of your SSN is voluntary. However, policyholder information is provided to the Federal Government for Flood Insurance programs and since many persons
appearing in administration records possess identical names, the use of your SSN would provide for your precise identification.
Schreiber will confirm information provided through consumer reports, which may include credit information. Upon request,
Schreiber will provide the source of these consumer reports and their customer service phone numbers for verification and correction of your information. Your credit information and your answers to questions asked by
Schreiber will be available to the insurance companies with which Schreiber is affiliated.
Some of these companies include Progressive Insurance Company, Safeco, and Union
Standard.
